As we work together to deliver integrated, proactive health and care, we will empower people, communities and partnerships to take collective ownership of local issues and help the people of Bedfordshire, Luton and Milton Keynes to stay well and receive joined-up, integrated support when they need it.

Data and expertise from all partners in Bedfordshire, Luton and Milton Keynes will help us to drive change by designing community-focused proactive care and deliver improvements in health and wellbeing, making best use of our collective resources. This is called Population Health Management.
