GP Connect helps care providers make safer, faster decisions

GP Connect is an NHS service that gives adult social care providers (registered with the Care Quality Commission) secure access to view key information from a person’s GP record in real time. It helps ensure the right people have the vital information they need to support safer, more personalised direct care – without relying on phone calls or waiting for email updates from GP practices.

With GP Connect, authorised care staff can access important parts of a GP record directly through the assured Digital Social Care Record (DSCR) system. This includes:

  • The last three encounters of interaction with professionals at the GP practice, including any summary information if this has been added to the record
  • Details of current and previous medications
  • Information on allergies and adverse reactions
  • Active concerns and medical history
  • Immunisation record
  • Test results and referrals (read-only where available).

Having this information can support care providers in planning and delivering their care more effectively. It reduces the time spent chasing information, such as changes to care plans. It supports better co-ordination and communication between care providers, GPs and health professionals which in turn can help with smoother hospital admissions and swifter discharges.

GP Connect is free to use, and available now via any of the assured DSCRs. The set-up process is seamless – there’s no extra work needed from the GP practices. Care providers just need to contact their DSCR supplier, who will guide them through the process of enabling GP Connect at their location.

For more information including videos, case studies and a step-by-step guide, see the GP Connect Resource Pack.

What care providers told the national GP Connect team:

“Before GP Connect, we were calling GP surgeries all day. Now we can check medications and notes instantly. It’s made everything quicker and safer – especially for new admissions or end-of-life care.” – Registered Manager
